The following is a checklist that must be completed before your appointment to the VA can be accomplished. If you have any questions, please contact Human Resources at 303-393-2815.

Before you begin your rotation at the VA you must complete items 1 through 5 on the following list:

• Infection Prevention and Control

• JCAHO Patient Safety Goals

• Medical Equipment Management

• No Fear Whistleblowing

• Security Management Plan

• Sexual Harassment

• Utilities Management

• HIPAA & Statement of Committment and Understanding (Last 2 pages of this document must be printed and submitted with packet)

___ 4. VA Appointment Affidavits letter form  (1st year and new Residents/Fellow ONLY)
    NOTE: Notary is not required. Leave Notary portion of form blank. The VA/HR Department will complete.

___ 5. Fingerprinting/Badging through VA HR - needs to be scheduled through your University Program Coordinator. 
    NOTE: You will need to bring two forms of identification to your appointment at the VA Acceptable Forms of ID.

Return completed WOC letter, Statement of Commitment and Understanding for VA Trainees, Clinical Trainee Registration Form and Appointment Affidavits (if required) to your UCHSC Program Coordinator. S/he will forward all necessary documents to VA Human Resources.
