Dictation Instructions and Guide for Dictation of Discharge Summaries and Operation Reports will be distributed at the resident orientation.

Delinquent list is sent out every Wednesday to the Service Chiefs who will contact the residents for completion of discharge summaries and operation reports.

HIPAA Privacy / Release of Information Requests
All requests for copies of patient's records must be sent to the Release of Information Department,
Room BA-121 or contact Mary Knudson x2080.
