Welcome to the VA Eastern Colorado Health Care System and the Denver VA Medical Center! We look forward to working with you and making your training a valuable experience.

The following is a checklist that must be completed before your appointment to the VA can be accomplished. If you have any questions, please contact Human Resources at 303-393-2815.

Before you begin your rotation at the VA you must complete items 1 through 4 on the following list:

___ 1. VA Mandatory Annual Training Modules & HIPAA - Click the links below. (In the WOC letter in #2 below, you will certify that you have completed these modules)

___ 2. WOC appointment letter form

___ 3. VA Clinical Trainee Registration Form

___ 4. Fingerprinting/Badging through VA HR - needs to be scheduled through your University Curriculum Office for signup schedule. 
    NOTE: You will need to bring two forms of identification to your appointment at the VA.  Click the link to review acceptable forms of identification: www.denver.va.gov/clinicaltrainee/pividproofingcriteria.pdf.

Bring completed WOC Appointment Letter, HIPAA Certification, Clinical Trainee Registration Form, and Appointment Affidavits (if necessary) to VA Human Resources office.
