Colorado VCP is excited to announce the return of Access to Care Rounds! Please join us for an interactive panel discussion involving care coordination for a WWII Veteran along with the perspective of the Veteran's daughter. Some of the programs and services that will be discussed include: the Veteran Directed Care Program, the VA Home Based Primary Care Program, DRCOG, and the Aid and Attendance Program.

 Veteran Community Partnership Initiative will Expand to Every VA Medical Center by 2024
VCP Colorado is one of 35 active VCPs across the country. By 2024 the goal is to have a VCP in every VA medical center. That means there will be 170 VCPs across the land!
VCPs are partnerships—and each is part of the larger VCP initiative—that bring community leaders, some of whom are Veterans, and organizations together with VAMC programs to help Veterans access health care and supportive services at VA and beyond. VCPs offer help in areas like palliative care and military discharge status, or hold celebratory events and household donation drives for Veterans, and much more.
