Medical Foster Home care, as part of the Home Based Primary Care team (HBPC), is an alternative to nursing home care and offers our Veterans 24/7 comprehensive long-term care in a safe, home-like environment. The HBPC team consists of Certified Nurse Practitioners, Nurses, Social Workers, Occupational Therapists, Physical Therapists, Dieticians, Psychologists and Recreational Therapists, each supplementing the Veteran's care in the Medical Foster Home.
